Position Summary

The RN Case Manager provides clinical leadership, supervision, and operational oversight for a team of Nursing Case Managers delivering services to individuals receiving care through OneWell Health Care. This role ensures high-quality, compliant, and person-centered case management services while supporting staff development, operational growth, and regulatory compliance within the state of Georgia.

Position Scope

This position functions as a Registered Nurse Case Manager within the Nursing Department, providing clinical oversight and coordination of care for assigned clients. The RN Case Manager is responsible for assessing client needs, developing and monitoring individualized care plans, coordinating services among interdisciplinary team members, and ensuring compliance with regulatory and company standards. The role supports quality assurance initiatives, facilitates communication between clients, caregivers, healthcare providers, and community resources, and promotes positive client outcomes through effective care management. Responsibilities include care coordination, clinical documentation review, ongoing client assessments, and participation in departmental quality improvement efforts.

Key Responsibilities

• Coordinate and oversee the delivery of care for assigned clients, ensuring services align with individualized care plans and identified needs.
• Conduct comprehensive nursing assessments and develop, implement, and monitor individualized care plans to promote positive client outcomes.
• Ensure compliance with state, federal, payer, and organizational requirements related to clinical documentation, care delivery, and case management activities.
• Participate in quality assurance and performance improvement initiatives, including chart reviews, audits, and corrective action planning as needed.
• Collaborate with caregivers, healthcare providers, interdisciplinary team members, and community resources to ensure continuity and coordination of care.
• Support nursing leadership and operational teams by providing clinical expertise, assisting with client transitions, and contributing to departmental goals and process improvement efforts.
• Support training and onboarding of new nursing staff
• Supervise, mentor, and evaluate all in-field employees to ensure effective and compliant case management practices
• Review, approve, and oversee care plans, assessments, and nursing documentation
• Coordinate with healthcare providers, patients, families, and support coordination entities to ensure continuity of care
• Partner with Nursing Operations to support audits, quality reviews, and corrective action plans
• Assist with recruitment efforts, including posting job opportunities, interviewing, and hiring nursing staff
• Support departmental growth initiatives, outreach, and revenue targets

Regulatory Frameworks

• Georgia Department of Health and applicable state nursing regulations
• Medicaid and waiver program requirements, as applicable
• HIPAA and patient privacy regulations
• Organizational policies, clinical standards, and quality assurance protocols
• Applicable federal healthcare regulations

Confidentiality

The RN Case Manager is responsible for maintaining strict confidentiality of all patient, employee, and organizational information. This includes compliance with HIPAA, state privacy laws, and OneWell confidentiality policies.

Documentation Requirements

• Ensure accurate and timely completion of nursing assessments, care plans, and case notes
• Maintain oversight of case management records and audit readiness
• Track staff performance documentation, corrective actions, and training records
• Prepare reports related to team performance, patient outcomes, and quality metrics

Certifications

• Active and unrestricted Registered Nurse (RN) license in the state of Georgia (required)
• CPR certification (required)
• Additional case management or leadership certifications preferred

Education

• Bachelor of Science in Nursing (BSN) required
• Master’s degree in Nursing, Healthcare Administration, or related field preferred

Experience

• Minimum of 2 years of nursing experience
• Prior experience in case management preferred
• Experience working with IDD, waiver services, or community-based healthcare preferred

Technical Skills

• Proficiency with electronic health records and case management systems
• Strong documentation and reporting skills
• Ability to analyze data, audits, and performance metrics
• Competence with standard office and clinical software platforms

Languages

English proficiency required. Bilingual skills are a plus and may be required based on service population. 

Driver's License

Valid driver’s license required. Ability to meet company driving and insurance requirements. 

Travel Requirement

Occasional travel within Georgia required for staff support, audits, meetings and patient-related coordination.
