Position Summary
The Registered Nurse (RN) Case Manager is responsible for coordinating, assessing, and managing nursing care for individuals with Intellectual and Developmental Disabilities (I/DD) and other healthcare needs in home-based and community settings. The role combines clinical assessment, case management, skilled nursing services, medication certification training, care coordination, and regulatory compliance to ensure safe, person-centered, and high-quality care. The RN Case Manager will initially support New Jersey operations through medication certification training for non-licensed Direct Support Professionals (DSPs), with additional home health nursing responsibilities implemented as services expand in New Jersey and Pennsylvania. This is a part-time position with remote or hybrid flexibility and required travel for in-home visits and clinical assessments.
Position Scope

The position collaborates with the Director of Nursing, clinical leadership, physicians, families, support coordinators, field nurses, and direct support staff to ensure appropriate care planning, continuity of services, and compliance with applicable clinical and regulatory standards. The position initially focuses on implementing and supporting medication certification training for non-licensed DSPs in New Jersey. As home health nursing services are established and expanded, responsibilities will include comprehensive patient assessments, physician-ordered skilled nursing care, clinical oversight, Plan of Care review, and quality assurance. The role may also support Pennsylvania operations through clinical review of Plans of Care and related documentation.

The role also ensures accurate clinical documentation, regulatory compliance, quality assurance, incident follow-up, and clinical oversight of field nursing staff while supporting staff development, operational reporting, and the growth of home health nursing services in New Jersey and Pennsylvania.

The role is coordinating and communicating with other team members, addressing concerns of patients and their families, and reviewing documentation and nursing notes. Every day is pretty different.
0:33
It's not one specific moment, but when everything we've done comes together — the coordinating — and we get to see how we made an impact on the patient's life or their family and made things easier, that's when you feel how rewarding it is to work here.

We all seem to have the same goal of making everyone's lives easier and helping the underserved. Having everyone directed toward that and working so well together is really impressive — we all share the same mission.

OneWell has supported my professional growth by giving me the opportunity to move up into leadership roles and work on that side of things, collaborating with the team.

The ability to grow in your position at this company is the biggest reason why I stay. There's a lot of flexibility and a lot of ways to do better and learn different things, which helps you not stay stagnant.
1:25
If you have the initiative to work with other people, learn new things, and the desire to help others, then I think this is a great job for you.

Being in the home gives you the advantage to slow down, take your time, and really make sure everything you're doing is meaningful and important. In hospitals you can get chaotic and lost in paperwork, so I think in-home is more desirable.
