Job Description
OneWell Health Care is seeking a compassionate Physical Therapist (PT) to provide personalized therapy to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings in Pennsylvania.
You will evaluate each individual's physical needs, develop goal-based treatment plans, and collaborate with caregivers and other professionals to improve mobility, strength, balance, safety, and overall independence.

As a Physical Therapist, You’ll…
- Evaluate mobility, strength, balance, gait, posture, and safety needs.
- Develop and update individualized treatment plans.
- Provide evidence-based physical therapy in home and community-based settings.
- Help individuals improve mobility, coordination, physical function, and independence.
- Recommend mobility aids, adaptive equipment, therapeutic exercises, and fall-prevention strategies.
- Educate individuals, families, and caregivers on safe mobility techniques.
- Collaborate with caregivers and interdisciplinary team members.
- Complete accurate and timely clinical documentation.
- Participate in required meetings and trainings.
What We're Looking For:
- Doctorate or Master's degree in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in the state of practice.
- Current CPR certification (preferred or required in accordance with organizational policy).
- Strong clinical, documentation, organizational, and communication skills.
- Proficiency in functional mobility and gait assessments, balance and postural control interventions, adaptive and assistive equipment recommendations, evidence-based physical therapy techniques, and electronic health record (EHR) systems.
- Valid driver's license, a clean driving record, reliable transportation, and willingness to travel to home and community-based service locations. Flexibility to support varying schedules and assigned service areas is required.
- Experience supporting individuals with Intellectual and Developmental Disabilities (IDD) and providing physical therapy services in home or community-based settings is preferred.
- English proficiency is required. Bilingual proficiency (including Spanish) is a plus.
