Position Summary

The Licensed Practical Nurse (LPN) provides skilled nursing services to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings throughout Pennsylvania. Working under the supervision of a Registered Nurse (RN) Case Manager, the LPN implements individualized care plans, administers medications, performs clinical procedures, and supports individuals and families in maintaining health, safety, dignity, and quality of life.

Position Scope

This position operates within the Nursing Department and reports directly to the RN Case Manager or Director of Nursing. The LPN delivers hands-on clinical care consistent with Pennsylvania regulations and OneWell Health Care standards. Services are provided in participant homes, community settings, and other approved service environments.

Key Responsibilities

The LPN is responsible for implementing individualized care plans, administering medications and treatments, monitoring health conditions, providing education to families and caregivers, maintaining accurate documentation, supporting infection control practices, and collaborating with interdisciplinary teams to ensure coordinated and compliant care delivery.

Secondary Duties

Support quality improvement initiatives.
Assist with internal audits or clinical compliance reviews.
Participate in professional development and required training programs.
Perform additional duties as assigned within scope of licensure.

Tasks

Administer prescribed medications and monitor for side effects or adverse reactions.
Perform clinical procedures including wound care, catheter care, feeding tube management, and vital sign monitoring.
Support individuals with chronic condition management (e.g., diabetes, epilepsy, respiratory conditions).
Observe and document physical, emotional, and behavioral changes.
Report changes in condition promptly to the RN Case Manager.
Participate in interdisciplinary meetings and case reviews.
Provide training and orientation to families, caregivers, and new nursing staff.
Maintain infection control and safety standards in all care environments.

KPIs

Accurate and timely medication administration (100% compliance).
Completion of required documentation within established timeframes.
Zero preventable medication errors.
Timely reporting of incidents and health status changes.
Compliance with Pennsylvania regulatory standards.
Positive participant and family satisfaction feedback.

Decision Making

The LPN exercises clinical judgment within the scope of practical nursing licensure. Independent decisions may be made regarding routine care implementation; however, changes to care plans, escalation of medical concerns, and complex clinical decisions must be reported to and coordinated with the supervising RN.

Supervisor Responsibility

This position does not have direct supervisory authority. The LPN may provide clinical guidance, training, or orientation to Direct Support Professionals (DSPs) or newly hired nursing staff as delegated by the RN Case Manager.

Confidentiality

The LPN is responsible for safeguarding all Protected Health Information (PHI) and confidential participant information in accordance with HIPAA, state law, and OneWell Health Care policies. Unauthorized disclosure of confidential information is strictly prohibited.

Documentation Requirements

Maintain accurate and timely clinical documentation in the designated electronic health record system.
Document all medications, treatments, assessments, and incidents.
Ensure documentation aligns with ISP requirements and nursing standards.
Complete reports in compliance with state and organizational timelines.

Certifications

Current and active Licensed Practical Nurse (LPN) license in Pennsylvania
CPR and First Aid certification (required)
Any additional certifications required by state or waiver programs

Education

Completion of an accredited Practical Nursing program approved by the Pennsylvania State Board of Nursing.

Experience

Minimum of one (1) year nursing experience required.
Experience supporting individuals with IDD, home health, or long-term care preferred.

Technical Skills

Proficiency in medication administration and clinical procedures.
Familiarity with electronic documentation systems.
Ability to use home-care medical equipment (feeding tubes, oxygen, suction equipment, etc.).
Basic Microsoft Office and mobile application proficiency.

Languages

Ability to communicate effectively in English, both verbally and in writing. Additional languages are considered an asset based on participant population needs.

Driver License

Valid driver’s license in good standing and reliable transportation required. Must meet OneWell Health Care motor vehicle eligibility requirements.

Travel Requirement

Local travel within assigned Pennsylvania service regions is required. Travel may include participant homes, community settings, and occasional attendance at meetings or trainings.
