Position Summary

The In-Home Caregiver provides personalized, non-medical support to individuals in their homes to promote safety, independence, dignity, and quality of life. This role assists with activities of daily living (ADLs), companionship, light household tasks, and wellness monitoring while following individualized care plans. The Caregiver plays a critical role in observing, documenting, and reporting changes in condition to ensure continuity of care.

Position Scope

The In-Home Caregiver delivers direct support services in private home settings. Services are non-clinical in nature and are provided under the supervision of an RN Case Manager or designated clinical leadership. The role requires independent work in community and home environments while maintaining adherence to organizational standards and care plans.

Key Responsibilities

The In-Home Caregiver is responsible for providing daily personal assistance, supporting health and wellness routines, maintaining a safe home environment, fostering social engagement, and ensuring accurate communication with supervisors and family members.

Secondary Duties

Participate in team meetings or trainings
Support new client onboarding routines
Assist with quality improvement initiatives
Perform other duties as assigned within scope of practice

Tasks

Personal Care Assistance

Assist with bathing, grooming, dressing, and personal hygiene
Support toileting and incontinence care
Assist with mobility and safe transfers (bed to chair, ambulation support)

Medication Support

Provide timely medication reminders
Assist with organizing pillboxes (non-clinical assistance only)
Observe and report potential side effects or concerns

Meal Preparation & Nutrition

Plan and prepare meals based on dietary needs
Encourage proper hydration and nutrition

Light Housekeeping

Perform basic cleaning (laundry, dishes, dusting, vacuuming)
Maintain safe and sanitary living conditions

Companionship & Community Engagement

Engage individuals in activities (reading, games, conversation)
Support community access and social participation
Assist in connecting individuals to community resources

Transportation & Errands

Drive or accompany individuals to appointments or outings (if required)
Run essential errands (e.g., groceries, prescriptions)

Health & Wellness Monitoring

Observe physical, behavioral, or emotional changes
Report concerns promptly to RN Case Manager or family
Maintain daily care logs and incident documentation

Communication & Coordination

Follow individualized care plans
Provide timely updates to supervisors and family members

KPIs

Incident reporting accuracy and timeliness
Maintenance of safe home environments

Decision Making

The In-Home Caregiver exercises judgment in daily care delivery within the limits of assigned care plans. Clinical decisions, medication administration changes, and care plan modifications must be escalated to the RN Case Manager or supervising clinician.

Regulatory Frameworks

Services must comply with applicable state home care regulations, Department of Health guidelines, Medicaid waiver requirements (if applicable), HIPAA standards, and organizational policies governing home and community-based services.

Confidentiality

The In-Home Caregiver must maintain strict confidentiality of all participant information in accordance with HIPAA and company privacy policies. Protected health information (PHI) must not be disclosed except as required for care coordination.

Documentation Requirements

Daily service logs
Incident reports (if applicable)
Care plan adherence notes
Mileage logs (if transportation provided)
Communication notes with supervisors/family

All documentation must be completed accurately and submitted in accordance with company policy and deadlines.

Certifications

Required:

CPR Certification

Preferred (if applicable by state):

Home Health Aide (HHA) certification
First Aid Certification

Education

High School Diploma or GED required.

Experience

Prior experience as a caregiver, home health aide, or similar role preferred
Experience supporting elderly or individuals with disabilities is highly desirable
Demonstrated ability to follow care plans and written instructions

Technical Skills

Ability to document electronically (mobile app or EMR system)
Basic understanding of personal care best practices
Safe transfer techniques
Time management and organization skills

Languages

Ability to communicate effectively in English (written and verbal). Additional languages preferred based on participant population.

Driver License

Valid Driver’s License required if transportation is part of assignment. Clean driving record required. Proof of auto insurance (if using personal vehicle).

Travel Requirement

Local travel required for participant appointments, errands, or community activities. Travel is primarily within assigned service area.
