Through coordinated care, advocacy, and support, we help individuals secure and sustain safe, affordable, and accessible housing aligned with their personal goals and quality of life.

Position Summary

The Housing & Tenancy Coordinator plays a vital role in helping individuals with IDD identify, obtain, and maintain appropriate housing within the community. This position ensures that all services provided align with the Housing Transition and Tenancy Sustaining Services (HTTSS) guidelines established by the Pennsylvania Office of Developmental Programs (ODP).

The coordinator works directly with individuals, families, landlords, and support teams to promote housing stability, independence, and inclusion—while ensuring compliance with all regulatory and programmatic requirements.

Key Responsibilities

1. Housing Transition Services

  • Develop individualized housing support plans tailored to each person’s needs, preferences, and resources.
  • Educate individuals about housing rights and responsibilities, including lease agreements, tenant obligations, and fair housing laws.
  • Support individuals in identifying appropriate housing options and completing housing applications.
  • Coordinate with landlords, housing authorities, real estate agents, and community agencies to secure suitable housing placements.
  • Assist with move-in logistics, including the purchase of household items, setting up utilities, and arranging transportation as needed.

2. Tenancy Sustaining Services

  • Help individuals build and maintain positive relationships with landlords and property managers.
  • Provide ongoing education on tenant rights, responsibilities, and communication strategies to support stable housing.
  • Assist with problem-solving and conflict resolution, addressing issues such as lease violations, disputes, or maintenance concerns.
  • Offer budgeting and financial planning assistance to promote long-term tenancy stability.
  • Collaborate with the individual’s broader support network—including family members, Supports Coordinators, and service providers—to ensure coordinated and holistic housing support.

3. Collaboration and Documentation

  • Participate in Individual Support Plan (ISP) meetings and team discussions when requested.
  • Maintain accurate and timely documentation in accordance with ODP and organizational requirements.
  • Coordinate effectively with Supports Coordinators, families, and housing stakeholders to ensure services are integrated and compliant with ODP waiver guidelines.
  • Ensure all activities promote independence, community participation, and person-centered service delivery.

Qualifications

Education

  • Bachelor’s degree in Human Services or a related field; equivalent experience may be accepted per ODP standards.

Experience

  • Minimum 1 year of experience supporting individuals with intellectual and developmental disabilities (IDD).
  • Familiarity with HUD programs, local housing authorities, and fair housing regulations.
  • Strong skills in organization, communication, and interpersonal engagement.
  • Ability to work independently while managing multiple cases and priorities.
  • Valid driver’s license and reliable transportation required for community-based work.

Preferred Qualifications

  • Experience in Housing First, supportive housing, or community-based residential services.
  • Knowledge of ODP waiver programs, particularly Consolidated, Community Living, or P/FDS waivers.
  • Certification or training in housing navigation, tenancy supports, or related housing assistance programs.

Work Environment

  • Community-based position requiring frequent travel to individuals’ homes, community settings, and housing providers.
  • Flexible scheduling may be required to accommodate client needs, housing availability, or landlord meetings.
  • Regular interaction with families, agencies, and other service systems to support housing stability.

I love working here because I feel like the work we're doing truly matters.

Hi, my name is Laura McBride and I've been with the company about six years. The work really revolves around the individual you're supporting — that could look like being a companion, someone who's there to spend time and make sure the individual feels supported and secure.

0:29

It's not defined by one specific moment, but anytime I see that we're able to make that happen for somebody — an individual or a family member who so desperately needs support — it's just a really magic moment.

0:37

What I love is our growth and our willingness to innovate — looking into the barriers we're seeing and asking how we can do this better.

0:50

I've been able to navigate multiple healthcare systems across many states. I stay because I believe in what we're building — I truly feel like OneWell has an opportunity to make a difference.

0:58

I would tell somebody to come in with an open mind and a willingness to learn. If you're looking for a position where you do the same thing every day, OneWell may not be the place for you.

1:15

But if you come in willing to adapt and grow with the individual you're serving, there's definitely a huge opportunity to make a big impact. It's all about making decisions that are in the best interest of the individuals you're caring for — those decisions could look a little different day to day, but they're mainly based around their primary needs and goals.

1:31

So being able to adapt, think quickly, and make the safest decision to make sure the individual's health and welfare is at the forefront.
