Position Summary

The Home and Community Support Professional (HCSP) provides direct support services to individuals with Intellectual and Developmental Disabilities (IDD) under Indiana Bureau of Developmental Disabilities Services (BDS) programs. This position delivers person-centered services that promote independence, community integration, daily living skills, health, safety, and overall well-being in accordance with each individual’s Person-Centered Individualized Support Plan (PCISP).

Position Scope

The HCSP is responsible for supporting individuals with daily living activities, personal care, behavioral support, community integration, health and safety monitoring, and skill-building activities while maintaining compliance with BDS requirements, company policies, and individualized support plans.

Key Responsibilities

The Home and Community Support Professional (HCSP) provides direct care and person-centered support services to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings. Responsibilities include assisting with personal care, meal preparation, mobility, daily living skill development, community participation, and implementation of individualized goals outlined in the Person-Centered Individualized Support Plan (PCISP). The HCSP monitors and reports health, behavioral, and safety concerns, supports behavioral interventions and de-escalation strategies, maintains accurate daily documentation and service notes, and collaborates with families, guardians, supervisors, and interdisciplinary team members to ensure quality care and compliance with company policies and Indiana BDS requirements.

Regulatory Frameworks

This position operates in compliance with:

• Indiana Bureau of Developmental Disabilities Services (BDS) requirements
• Individual Person-Centered Individualized Support Plans (PCISP)
• Applicable federal, state, and local regulations
• Company policies and procedures
• Infection control and incident reporting requirements

Confidentiality

The HCSP is responsible for maintaining confidentiality of participant information, medical information, behavioral information, and organizational records in accordance with company policies and applicable privacy requirements.

Documentation Requirements

• Complete daily service notes for each shift
• Maintain accurate records regarding participant activities, progress, incidents, and health observations
• Document behavioral incidents and interventions
• Submit documentation within required timelines
• Maintain documentation in accordance with company policy and BDS requirements

Certifications

• CPR Certification required
• Completion of BDS required HCSP training modules prior to providing services
• Negative TB test required
• Criminal Background Check required
• Federal and State Exclusion Checks required

Education

High school diploma or equivalent required.

Experience

Prior experience in caregiving or direct support services for individuals with IDD preferred.

Technical Skills

• Ability to complete electronic documentation and daily service note submissions
• Ability to maintain accurate records and reports
• Understanding of incident reporting processes
• Understanding of infection control and universal precautions

Languages

Strong verbal and written communication skills required. Ability to effectively communicate with individuals, families, guardians, supervisors, and interdisciplinary teams.  

Driver License

Valid Driver’s License and acceptable Motor Vehicle Check required, as applicable.

Travel Requirement

Travel within assigned service areas and community locations may be required based on participant needs and assigned services. Flexibility to work evenings, weekends, and overnight shifts may be required depending on operational needs.
