Position Summary

The Coordinated Family Support (CFS) Provider plays a crucial role in supporting adults with Intellectual and Developmental Disabilities (IDD) who choose to live in their family homes. This position focuses on empowering individuals to lead self-directed lives while providing ongoing, person-centered support that respects their family dynamics, cultural values, and personal goals. The CFS Provider works to strengthen family systems, promote independence, and enhance community inclusion.

Key Responsibilities

Individual and Family Support

• Assist individuals in achieving person-centered goals related to health, safety, education, employment, and social participation.
• Provide guidance and advocacy to help individuals and families navigate healthcare systems, public benefits, transportation, and other community resources.
• Coach individuals and family members to enhance independence, self-determination, and decision-making skills.

Service Coordination and Communication

• Facilitate collaboration among individuals, families, regional centers, and service providers to ensure comprehensive and coordinated support.
• Support the development and implementation of individualized service plans tailored to each person’s needs and goals.
• Monitor individual progress and well-being, promptly reporting significant changes or concerns to supervisors or care teams.

Documentation and Compliance

• Accurately document services provided, progress toward goals, and any relevant updates in a timely manner.
• Uphold confidentiality and data protection standards in full compliance with HIPAA regulations.

Community Engagement

• Assist in arranging and attending community activities, appointments, and events to promote inclusion and participation.
• Maintain professional, respectful relationships with individuals, families, and community partners.

Qualifications

• Bachelor’s degree (required); additional education in social work, psychology, or human services is preferred.
• Prior experience supporting individuals with developmental disabilities and/or family-based services is desirable.
• Strong interpersonal, communication, and organizational skills.
• Ability to work independently while maintaining collaboration within a multidisciplinary team.
• Valid driver’s license and reliable transportation required for community-based work.
• Successful completion of a background check required.
• Flexibility, cultural sensitivity, and reliability are essential.

Working Conditions

• CFS Providers primarily work in clients’ homes and community-based settings.
• Flexibility in scheduling is required, with occasional evening or weekend hours.
• The role involves local travel for meetings, visits, and support activities.
